CASE STUDY:
J.D. is a 67-year-old Male who presents to the emergency room via ambulance one hour after beginning to feel heaviness in his left arm and difficulty grasping his cellphone. He reports having a history of high blood pressure and high cholesterol for which he is taking amlodipine 10 mg/day and atorvastatin 40mg/day. When asked if he takes any blood thinning medication, he replies no. He denies family history of stroke and has no known allergies. He denies use of illicit substances, alcohol, and tobacco. The National Institute of Health (NIH) stroke scale was rated as a 2.
The patient(J.D.) is diagnosed as having a stroke and is taken to have a CT angiogram (CTA) of the head and neck. The CTA findings show a characteristic filling pattern known as a carotid web (otherwise called septum or shelf-like deficit) that originates from the posterior wall of the right internal carotid artery. The left carotid artery shows normal vasculature. He is offered treatment with intravenous thrombolysis however declines treatment due to improvement in his condition and with only mild deficits against baseline. J.D. further undergoes MRI of the brain which shows an acute infarct in the distribution of the right internal carotid artery. MRI further reveals previous signs of a stroke in the form of encephalomalacia within the same distribution of the current embolic event.
